PRINT REGISTRATION / INFORMATION FORM

WHERE:       Perfect North Slopes, Lawrenceburg, Indiana

WHEN:         December 28-30, 2009

LEAVING:    Monday, 12/28  3:00 PM

RETURNING:  Wednesday, 12/30 11:59 PM 

Departure and arrival will be from/to the College Hills church building.

LODGING:  Holiday Inn Express, Lawrenceburg, Indiana

COST:  The cost for the trip is only $190, which includes lift tickets and equipment rental for two days, a beginner’s ski lesson, ski tubing, transportation, two meals, and two nights lodging.  Extra money will be needed for five meals. The five meals you’ll need money for will be supper on Monday, lunch and supper on Tuesday, and lunch and supper on Wednesday.


WHAT TO BRING: 

1. Toboggan or other warm head covering      

2.  Water-Proof Gloves

3.  Ski-bibs or other water-resistant pants 
(Remember that synthetic materials help to keep moisture away from your skin. Do not wear cotton as a base or outer layer.  Synthetic or wool works best.)

4.  Goggles and/or sunglasses

5.  Warm Jacket or parka

6.  Warm, dry socks      

7.  Boots for walking to the slopes

8.  Sunscreen and lip balm

9.  Snacks (fruit or energy bars)

10. Identification Card

11. Dry clothes to change into

12. Modest swimwear for indoor pool

13. Toiletries (soap, deodorant, etc.

 

SKI TRIP SCHEDULE

MONDAY, DECEMBER 28

       3:00 – Leave College Hills  

       9:30 – Arrive and Unpack

     10:00 – Devotional at Hotel

     11:00 – Be in Rooms

     11:30 – Lights Out for Rest!

TUESDAY, DECEMBER 29

       7:30 – Breakfast at Hotel

       8:30 – Leave for Slopes     

                  Ski!

     12:00 – Lunch at Slopes

                  Ski!

       6:00 – Leave Slopes.  Return to Hotel

       7:00 – Dinner

     10:00 – Devotional

     11:00 – Be in Rooms

     11:30 – Lights Out for Rest!

 

WEDNESDAY, DECEMBER 30

     7:30 – Breakfast at Hotel

     8:00 – Check Out of Hotel

      8:30 – Leave for Slopes

                 Ski!

    12:00 – Lunch at Slopes

                 Ski!

      6:00 – Leave Slopes for Home

      7:00 – Dinner

    11:59 – Arrive at College Hills 

 

REGISTRATION INSTRUCTIONS
Fill out both sides of the page with the  Registration and
Permission Form, and turn it into the church office, or
to one of the Youth Ministry staff, on or before December 13.
The $190 fee is due upon submission of your
Registration Form.
